- Vendor with table/chair rental: $100 fee + $20 rental = $120 payment

Thank you for joining us at the 2016 Spring Carnival! Please note the following conditions:
Sponsors: Donations to PS 198M PTA, a 501(c)(3) non-profit organization, are fully tax deductible to the extent allowable by law. Sponsors will receive promotion benefits. Any sponsors wishing to sell goods must also follow vendor requirements below.
Vendors: Vendor fee entitles you to 10 ft. front footage space in our E. 95 St. street fair section. No power/water hook-ups available. Table/chair rental includes (1) 6 ft table and (2) folding chairs. Vendors must have a Temporary Street Fair Vendor license issued by the Dept. of Consumer Affairs (DCA) and valid tax ID. Food vendors must also have applicable food licenses which may include a “Temporary Food Service Establishment Permit” from the Dept. of Health, and a Food Protection Certificate. See www.nyc.gov for requirements. Vendors are required to show proof of applicable licenses and to return this completed Carnival vendor form to PS 198M PTA.
